Geriatric Care Management

If an older adult needs help with everyday activities, and you are unsure of how to handle medical, physical, social, emotional, and financial issues, our Geriatric Care Manager may be able to help you.

Our program is especially beneficial for adult children in other parts of the country who have an ageing family member living in SE Michigan, elder law attorneys, trust officers, older adults who live alone and have little or no support system, senior housing communities seeking adjunct services, discharge planners, and physicians. Our services include assessments, care planning, and care management.

Traditional Private Duty Home Care Services

Team of health care professionals are available to assist you with medical and non-medical needs, including personal care, companionship, skilled nursing, therapy, and home support activities, from one hour of service to around – the – clock care.

End of Life Care Program

We work closely with the patient, family, hospice and/or personal physician incorporating traditional concerns of hospice, and helping patients live as fully and as comfortably as possible. We provide in – home treatments, administration of medications, pain management; respite care, personal care, companionship and skilled nursing.

Alzheimer’s/Dementia Care

Our medical and non – medical services enable a person to stay at home and maximize independence for as long as possible. Our specially – trained caregivers promote each client’s capabilities, and minimize those stresses that can aggravate the symptoms of dementia and Alzheimer’s disease. We provide in – home respite, homemaking assistance, companionship, and safety assurance.
